失能老人家庭照顾者的照顾压力研究
——以南京市为例
摘要 作为当前失能老人基本生活照顾的主体,家庭照顾者(主要包括配偶、子女、亲友)承担了沉重的照顾压力,这些压力对他们的身心、工作、社交产生了深刻的影响。文章利用江苏省南京市失能老人家庭照顾者的调查数据,分析了失能老人家庭照顾者的照顾现状、照顾压力水平与来源以及照顾者对协助支持的需求程度,并深入研究了配偶、子女与亲友的照顾压力的影响因素。研究发现:家庭照顾者提供基本照顾的时间受到老人失能程度的影响较大;他们的照顾压力主要体现在生理、心理与经济方面,总体负担程度处于中等水平。在照顾压力的影响因素方面,除了失能老人和照顾者年龄、婚姻、经济收入之外,照顾时间、生理负担、经济负担、心理负担均对照顾者的照顾压力产生显著的影响,而且当前照顾者对家庭成员的协助照顾、政府提供的经济补贴与照护服务的需求迫切。

Abstract: As the subject of caring for the basic life of disabled elderly, family caregivers (including spouse, children, relatives and friends) suffer the heavy pressure. The pressures profoundly impact their mind and body, work and social contact. Using the survey data of disability elderly caregivers from Nanjing city, Jiangsu province, combined with statistical methods, this article analyzes the current situation of caregivers, the sources and levels of care pressures, the needs of family members and social support to release the pressure, and the factors associated with the pressures. According to the analysis, it reveals that the caring time of the family caregivers is affected by the disability levels of the elderly. Their care pressures are on the aspects of physical, psychological, and financial and the overall pressure level is at a moderate level. In terms of influencing factors of care pressures, besides disability elderly and caregivers age, marriage status, economic income, caring time, physiological burden, financial burden and psychological burden also have a significant impact on caregivers. Moreover, caregivers urgently need the support from family members and the government providing financial subsidies and care services.

人口老龄化形势严峻,速度惊人,成为世界上很多国家的人口问题。据国际助老会发布的《2015年全球老龄事业观察指数》显示,全球约有9.01亿的60岁以上老人,占当年全球总人口的12.3%,预计2030年这一比例将达16.5% 。作为人口大国,中国的老龄化问题也十分突出,在绝对数量上,2014年末中国60周岁以上老人占总人口的15.5%,达到2.12亿人 。在这部分老年人中,一个非常重要的群体——失能老人,其绝对数量也在逐年攀升,2010年末中国完全失能和部分失能老人约3300万人(占老年人口总体的19%) ,2012年这一比例上升到3600万人,2013年进一步上升到3750万人 。截至2014年底,我国失能、半失能老人高达4千万人 。
